Privacy Policy
Effective date: September 27, 2026
This Privacy Policy explains how Homefront Health & Hormones LLC ("Homefront," "we," "us") collects, uses, shares, and protects information when you visit homefronthormones.com, contact us, or enroll in care. Homefront is a management services company. Medical care is provided by independently licensed clinicians of an affiliated professional medical group (the "Practice").
Your health information is also protected by HIPAA. Protected health information created or received in the course of your care is handled as described in our Notice of Privacy Practices. Homefront handles that information on the Practice's behalf as a HIPAA business associate. If this policy and the Notice of Privacy Practices ever conflict about your health information, the Notice of Privacy Practices controls.

Information collected automatically on this website
Basic server logs needed to run and secure the site, and privacy-first analytics described below. We do not collect this information on the patient portal for analytics or advertising.
Tracking technologies
This website uses one analytics tool, Plausible Analytics. It does not use cookies, does not store anything on your device, and cannot follow you to other websites. For each page view it receives the page address, the referring site, and your screen width. It also counts, in aggregate, clicks on program and add-on links so we can see which programs people are interested in. Your IP address and browser information are used only to produce a daily visitor count and approximate country and device type, and are not stored.
There are no advertising pixels, social media pixels, session-recording tools, or cross-site trackers anywhere on this website, and none on intake forms, scheduling, or the patient portal. Because we do not sell or share information for targeted advertising, there is nothing to opt out of, but we honor Global Privacy Control and similar browser signals as an opt-out request.
How we use information
- To provide, coordinate, and support your care with the Practice, labs, and pharmacies.
- To confirm eligibility, including your age and the state where you are located.
- To process payments and manage your membership.
- To communicate with you about appointments, labs, prescriptions, shipping, and your account.
- To operate, secure, and improve our services and website.
- To comply with law, respond to lawful requests, and protect the rights and safety of patients and others.
How we share information
We share information only as needed to provide your care and run our services:
- The Practice and its licensed providers, who evaluate and treat you.
- Laboratories and licensed pharmacies involved in your testing and prescriptions.
- Service providers that host our patient platform and health records, process payments, and send email and text messages. Each is bound by a written agreement and, where it handles health information, a HIPAA business associate agreement.
- Others as required or permitted by law, as described in the Notice of Privacy Practices.
- A successor organization in a merger, acquisition, or sale of assets, subject to this policy and applicable law, including HIPAA.
We do not sell your personal information or health information, and we do not share it with advertisers or data brokers.
Email and text messages
If you give us your phone number, we may text you about your care and account. Message and data rates may apply. Reply STOP to opt out of non-essential texts at any time. Standard email and text messages are not fully encrypted. If you ask us to communicate that way, we will, but you can request a more secure method instead. We send marketing messages only with your consent, and you can opt out at any time.
Your rights and choices
Depending on where you live, you may have the right to:
- Know what personal information we hold about you and get a copy of it.
- Correct information that is inaccurate.
- Delete information, subject to legal requirements to keep medical and prescription records.
- Withdraw consent you have given us.
- Opt out of marketing communications.
- Appeal a decision we make about your request.
Rights to your medical records, including access, amendment, restrictions, and an accounting of disclosures, are described in the Notice of Privacy Practices.
To make a request, email privacy@homefronthormones.com or call or text (402) 909-4936. We will verify your identity before acting on a request and respond within 45 days, or tell you if we need more time as the law allows. You may use an authorized agent. We will not discriminate against you for exercising your rights.
Consumer Health Data Privacy Policy
This section applies to "consumer health data" as defined by the Washington My Health My Data Act, Nevada law (SB 370), Connecticut law, and similar state laws. It covers health-related information that is not protected health information under HIPAA, such as the fact that a website visitor clicked on a specific program link.
- Categories we collect: information you share about your health goals or symptoms before becoming a patient, and aggregate records of interest in specific programs, such as testosterone therapy, hormone therapy, or weight-management medications.
- Sources: you directly, and our website analytics as described above.
- Purposes: to respond to you, to help you enroll, and to understand which programs people are interested in.
- Sharing: only with the service providers described above, under contract. We do not sell consumer health data.
- Your rights: to confirm whether we collect, share, or sell your consumer health data, to access it and a list of third parties and affiliates it has been shared with, to withdraw consent, and to have it deleted.
To exercise these rights, contact us as described above. If we deny your request, you may appeal by replying to our decision. We will respond to an appeal within 45 days. If your appeal is denied, you may contact your state's attorney general, including the Washington State Attorney General at atg.wa.gov.
Children
Our services are only for adults 18 and older. We do not knowingly collect personal information from anyone under 18. If you believe a minor has provided us information, contact us and we will delete it.
Security and retention
We protect information with encryption in transit and at rest, access controls, and security review of our service providers. No system is perfectly secure. We keep medical and prescription records for as long as the law of the state where you were treated requires, and other information only as long as needed for the purposes described here.
Changes to this policy
If we change this policy, we will post the new version here with a new effective date and notify active members by email of any material change.
