Terms of Use & Membership Agreement

1. What you're buying
Homefront provides access to licensed healthcare providers, laboratory testing, and — where clinically appropriate and prescribed by your provider — medication fulfilled by a licensed pharmacy. You are purchasing an evaluation and ongoing clinical services. You are not purchasing a prescription. No prescription is guaranteed, and fees are not contingent on receiving one.
2. Eligibility
You must be at least 18 years old, physically located in a state where Homefront's providers are licensed at the time of each visit, and able to provide accurate medical information. Providing false or incomplete health information may make treatment unsafe and is grounds for termination of your membership.
3. Fees, billing and automatic renewal
- Your first month includes a labs-and-consultation fee that brings your total first payment to $299 on an individual plan, or $499 on the couples plan. Every month after, you're billed your plan's normal monthly rate.
- Memberships are $199/month (Him), $209/month (Her), or $349/month (both partners). Semaglutide is $139/month and Tirzepatide is $149/month. Add-ons are billed at the rates shown when added.
- Your membership renews automatically each month and your payment method will be charged on the same day each month until you cancel. We'll email a receipt for every charge.
- Annual prepayment is billed once for twelve months at the price of ten.
4. Cancellation and refunds
- Cancel anytime through your account or by emailing us. No cancellation fee, no minimum term. We'll confirm in writing.
- Cancellation stops future charges. Charges already processed for the current month aren't refunded; you keep access through the end of that month.
- The first-month labs-and-consultation portion of your payment is non-refundable once lab testing is performed or a provider has reviewed results.
- Annual prepayments are refunded pro rata if you discontinue for a medical reason.
- Shipped medication can't be returned or refunded, per pharmacy law.
5. Nature of the relationship
Homefront Health & Hormones LLC is a management services organization. It does not practice medicine, does not employ the clinicians who treat you, and does not direct or control their clinical judgment. Clinical services are provided by independently licensed professionals of an affiliated professional entity. Providers receive no financial incentive tied to prescribing.
6. Not emergency care
Homefront does not provide emergency, urgent, or after-hours care. Call 911 or go to the nearest emergency department for emergencies. Call or text 988 for a mental health crisis. Maintain a relationship with a primary care provider.
7. Your responsibilities
Complete required lab testing on schedule, attend follow-ups, report side effects promptly, take medication only as prescribed, store it safely, and never share it. Sharing a controlled substance like testosterone is a federal crime.
8. Content and testimonials
Educational content here is general, not tailored to you. Any patient story published is from a real patient with written authorization, with any material connection to Homefront disclosed. Individual results vary.
9. Limitations, disputes, and governing law
Limitation of liability. To the fullest extent permitted by law, the total liability of Homefront Health & Hormones LLC for any claim arising out of your membership or your use of this site is limited to the amount you paid Homefront in the twelve months before the claim arose, and Homefront is not liable for indirect, incidental, special, or consequential damages. This limitation does not apply to liability that cannot be limited under applicable law, and it does not limit any claim concerning the professional judgment of your treating provider, which is governed by the law that applies to that provider.
Indemnification. You agree to indemnify Homefront against claims, losses, and costs that result from health information you knowingly misrepresent, or from your misuse, resale, or sharing of medication prescribed to you.
Resolving disputes. If you have a concern, contact us first at hello@homefronthormones.com or (402) 909-4936 and we will work in good faith to resolve it within 30 days. A dispute that isn't resolved informally may be brought in the state or federal courts located in Nebraska, unless the law of the state where you live gives you the right to bring it there.
Governing law. These terms are governed by the laws of the State of Nebraska, except where the consumer protection law of the state where you receive care requires otherwise.
Severability and entire agreement. If any part of these terms is found unenforceable, the rest remains in effect. These terms, together with the Privacy Policy, Notice of Privacy Practices, and Telehealth Informed Consent, are the entire agreement between you and Homefront regarding your membership.
10. Changes
We may update these terms and will notify active members by email at least 30 days before any change to pricing or membership terms takes effect.
