Homefront provides access to licensed healthcare providers, laboratory testing, and — where clinically appropriate and prescribed by your provider — medication fulfilled by a licensed pharmacy. You are purchasing an evaluation and ongoing clinical services. You are not purchasing a prescription. No prescription is guaranteed, and fees are not contingent on receiving one.
You must be at least 18 years old, physically located in a state where Homefront's providers are licensed at the time of each visit, and able to provide accurate medical information. Providing false or incomplete health information may make treatment unsafe and is grounds for termination of your membership.
Homefront Health & Hormones LLC is a management services organization. It does not practice medicine, does not employ the clinicians who treat you, and does not direct or control their clinical judgment. Clinical services are provided by independently licensed professionals of an affiliated professional entity. Providers receive no financial incentive tied to prescribing.
Homefront does not provide emergency, urgent, or after-hours care. Call 911 or go to the nearest emergency department for emergencies. Call or text 988 for a mental health crisis. Maintain a relationship with a primary care provider.
Complete required lab testing on schedule, attend follow-ups, report side effects promptly, take medication only as prescribed, store it safely, and never share it. Sharing a controlled substance like testosterone is a federal crime.

Limitation of liability. To the fullest extent permitted by law, the total liability of Homefront Health & Hormones LLC for any claim arising out of your membership or your use of this site is limited to the amount you paid Homefront in the twelve months before the claim arose, and Homefront is not liable for indirect, incidental, special, or consequential damages. This limitation does not apply to liability that cannot be limited under applicable law, and it does not limit any claim concerning the professional judgment of your treating provider, which is governed by the law that applies to that provider.
Indemnification. You agree to indemnify Homefront against claims, losses, and costs that result from health information you knowingly misrepresent, or from your misuse, resale, or sharing of medication prescribed to you.
Resolving disputes. If you have a concern, contact us first at hello@homefronthormones.com or (402) 909-4936 and we will work in good faith to resolve it within 30 days. A dispute that isn't resolved informally may be brought in the state or federal courts located in Nebraska, unless the law of the state where you live gives you the right to bring it there.
Governing law. These terms are governed by the laws of the State of Nebraska, except where the consumer protection law of the state where you receive care requires otherwise.
Severability and entire agreement. If any part of these terms is found unenforceable, the rest remains in effect. These terms, together with the Privacy Policy, Notice of Privacy Practices, and Telehealth Informed Consent, are the entire agreement between you and Homefront regarding your membership.
We may update these terms and will notify active members by email at least 30 days before any change to pricing or membership terms takes effect.

Your provider evaluates and treats you using video, audio, and secure messaging rather than an in-person visit. Your provider is licensed in the state where you are physically located at the time of the visit.
Access to a specialist without travel, appointments that fit around work and family, and continuity of care through secure messaging between visits.
You may withhold or withdraw consent to telehealth at any time without affecting future care. You may request an in-person referral at any time, and you have access to your records on the same terms as an in-person practice.
Testosterone is a Schedule III controlled substance. Prescribing controlled substances via telehealth is governed by federal law. Current federal rules allow it without a prior in-person visit through December 31, 2026, while a permanent framework is finalized. Your provider will check your state's prescription drug monitoring program. If federal rules change in a way that affects your treatment, we'll notify you in advance and help arrange an in-person evaluation where required.
Telehealth is not for emergencies. Call 911 or go to the nearest emergency department. We record your physical address at each visit so emergency services can be directed to you if needed.
By signing at intake, you confirm you've read and understood this consent, had the chance to ask questions, and consent to receive care by telehealth.
